UCD POLICY & PROCEDURE MANUAL Transmittal Notice 10/31/96

The attached UCD Policy & Procedure Manual sections have been approved by the appropriate vice chancellor and by the Provost and Executive Vice Chancellor. After inserting these sections in your manual, please annotate the Table of Contents to reflect the changes, then post or circulate this transmittal notice so employees will be aware of these policies. SUPERSEDES: Section 200-40, dated 3/30/94. PURPOSE: Includes clinical psychologists and podiatrists among those whose professional activities are governed by policies developed by the Medical Staff; updates references to former Executive and MAP Programs; updates names of Medical Staff Committees; clarifies that the Hospital & Clinics, Medical Staff, and School of Medicine are referred to as the UCD Health System (UCDHS). 2. TRANSMITTED: Section 290-50, Protective Clothing and Equipment. SUPERSEDES: Section 290-50, Protective Clothing, Equipment, and Uniforms, dated 9/23/91. PURPOSE: Deletes information regarding uniforms, which is available in the Personnel Policies for Staff Members; specifies protective clothing and equipment that may be required in laboratories; provides for EH&S review of department determinations for required protective clothing; deletes approval requirement by Associate Vice Chancellor-- Human Resources & Risk Management for a department to provide nonmandatory protective clothing and equipment, provided that cost limits are established to assure equitable treatment of employees with similar duties; deletes need for written specifications from EH&S prior to departmental purchase of mandatory protective clothing or equipment; updates references; editorial revisions for clarity. 3. TRANSMITTED: Section 290-95, Pesticide Applications. SUPERSEDES: Section 290-95, dated 11/10/89. PURPOSE: Removes requirement for department to file with EH&S the Notice of Intent to apply pesticides (instead, the department files Monthly Summary Pesticide Use Report directly with County Agricultural Commissioner); for application by aircraft, requires department to notify departments with property adjacent to the area to be treated; includes responsibility of department to assure current training of employees, as required by EPA, and to submit monthly Medical Surveillance form for employees handling certain types of pesticides; specifies that departments must have a permit from County Agricultural Commissioner to apply restricted materials; updates information on research and experimental applications; editorial revisions for clarity. 4. TRANSMITTED: Section 370-20, Workers' Compensation. SUPERSEDES: Section 370-20, dated 5/10/90. PURPOSE: Streamlines language and removes duplication of information available in personnel manuals; provides for coverage of volunteers; eliminates forms (personal physician packet, department injury/illness worksheet); simplifies instructions for Employer's Report and Employee's Claim forms; describes and simplifies modified duty program (formerly light duty program, as described in personnel manuals); updates unit names and addresses; updates references. 5. TRANSMITTED: Section 380-70, Independent Consultants, and Exhibits A-C. SUPERSEDES: Section 380-70 and Exhibits A-C, dated 10/5/89. PURPOSE: Requires comparison, by someone other than the person responsible for final payment approval, between terms of an independent consultant agreement and the evidence that the terms have been completed, consistent with UC Business & Finance Bulletin BUS-34; updates procedures; updates position titles and unit names.
