UCD POLICY & PROCEDURE MANUAL Transmittal Notice 8/5/98

The attached UCD Policy & Procedure Manual sections have been approved by the appropriate vice chancellor and by the Provost and Executive Vice Chancellor. After inserting these sections in your manual, please annotate the Table of Contents to reflect the changes, then post or circulate this transmittal notice so employees will be aware of these policies. TRANSMITTED: Section 200-06, 1998-99 Academic and Administrative Report Deadlines, dated 7/1/98. SUPERSEDES: New section. (Previous years' lists may be discarded/recycled as they become outdated.) PURPOSE: Provides UCD personnel with list of deadlines for various academic, administrative, and fiscal reports required during 1998-99 so they may better plan for possible workload fluctuations. 2. TRANSMITTED: Section 210-05, Affiliation Agreements, and Exhibit A, dated 7/10/98. SUPERSEDES: Section 210-05 and Exhibit A, dated 6/25/93. (DISCARD/RECYCLE.) PURPOSE: Clarifies required approvals; specifies that agreements covering rotation and/or preceptorship of non-UCD non-physician trainees at a UCDHS facility must be approved according to UCDHS policy/procedures; describes separate request forms for School of Medicine departments and for UCDMC departments and Primary Care Network clinics (Exhibit A). 3. TRANSMITTED: Section 290-09, Violence in the Workplace, dated 5/26/98. SUPERSEDES: New section. PURPOSE: Describes policy for preventing and responding to disruptive, threatening, or violent behavior involving any member of the UCD community, as well as guidance for protecting the UCD community from, and responding to, such conduct when directed toward them by unaffiliated visitors. 4. TRANSMITTED: Section 290-10, Smoke-Free Policy, dated 6/30/98. SUPERSEDES: Section 290-10, dated 10/15/93. (DISCARD/RECYCLE.) PURPOSE: Clarifies that smoking in outdoor passageways to building entrances is prohibited; simplifies procedures; updates references; editorial revisions for clarity. 5. TRANSMITTED: Section 290-45, Pest Management, dated 6/2/98. SUPERSEDES: Section 290-45, dated 6/27/89. (DISCARD/RECYCLE.) PURPOSE: Reflects change in responsibility for pest management program from Environmental Health & Safety to Custodial Division; provides for services through a vendor contract; updates procedures for pest control services, including telephone numbers for campus and UCDHS departments to request pest control. 6. TRANSMITTED: Section 290-55, Biological Safety, dated 7/8/98. SUPERSEDES: Section 290-55, Biological Safety, dated 10/15/93. (DISCARD/RECYCLE.) PURPOSE: Deletes definitions; reflects change in responsibility for Biological Safety Administrative Advisory Committee to Vice Chancellor--Administration; specifies responsibility of principal investigator to assure compliance with Occupational Health Physician recommendations for projects; requires Biological Use Authorization for activities involving use of human or nonhuman primate body fluids or tissue; updates references; editorial revisions for clarity. 7. TRANSMITTED: Section 290-86, Hazardous Operations, dated 7/8/98. SUPERSEDES: Section 290-86, Control of Physically Hazardous Operations, dated 9/23/91. (DISCARD/RECYCLE.) PURPOSE: Adds as physically hazardous operations those operations requiring lockout/tagout, fall protection, and trenching and shoring, and defines procedures and responsibilities for each; requires a confined space entry permit prior to each entry, unless the space is temporarily reclassified by authorized personnel; requires supervisors to perform periodic inspections of forklifts to assure Cal/OSHA operational requirements are met; updates references; editorial revisions for clarity. 8. TRANSMITTED: Section 380-40, Death of an Employee, dated 6/1/98. SUPERSEDES: Section 380-40, dated 9/25/87. (DISCARD/RECYCLE.) PURPOSE: Clarifies that this section applies to all UCD employees, regardless of work location; specifies that department head must notify the Workers' Compensation Unit if an employee's death occurred during the course of employment; updates unit names and position titles; updates references; minor editorial revisions. 9. TRANSMITTED: Section 380-57, Emergency Payroll Payments to Employees, dated 6/3/98. SUPERSEDES: Section 380-57, dated 10/3/88. (DISCARD/RECYCLE.)
